Worker Voice — surveys and the suggestions & complaints channel

Worker participation (clause 5.4) is what most distinguishes ISO 45001 from other standards, and what inspectors scrutinise hardest: having a safety committee is not enough — every worker's voice must reach management and be answered.

One tool, two halves

  • Surveys: ask everyone a specific question and measure the result.
  • The voice box: an always-open channel for suggestions and complaints together — because in practice the suggestions window and the complaints register are the same thing.

Confidentiality is structural, not a promise

When a worker chooses "submit anonymously", the submission is stored with no identifier at all — there is no name hidden by the interface yet sitting in the database. The same holds for anonymous survey answers: participation is recorded in a separate table that prevents answering twice, with no path linking an answer to its author. Even the submitter does not see their own anonymous submission in their list — and that is deliberate.

Without that strictness the "channel" becomes a trap: one worker complains, is identified, and everyone else falls silent.

Where workers reach it

  • "My Voice" in the sidebar: open surveys they have not answered, the suggestion/complaint composer, and their named submissions with management's replies.
  • The site wall when scanning a QR code: they answer the survey in place and write a suggestion or complaint without leaving the page — a worker standing at the board should not be asked to open other pages (see QR site profile).
  • An organization-wide notification reaches every worker the moment a survey opens.

The survey lifecycle

Draft, where you author the questions (1–5 scale, yes/no, free text) → Open, which fires the organization-wide notification → Closed, where results are read.

Composition freezes with the first answer: no question may be added, edited, or have its anonymity promise flipped afterwards — the answers were measured against one specific composition, and changing it falsifies the totals. Need a change? A new survey.

The results page shows the participation rate, the average and distribution of each scale question, yes/no shares, and free-text answers stripped of any identity.

Triage — and the most important feature

In the voice box a submission passes through four states: New → In review → Answered → Closed.

A submission cannot become "answered" while the response field is empty — a deliberate gate: closing without replying is exactly what makes workers lose faith in the channel.

Once the reply is written, a notification reaches the named submitter in their bell inbox ("management has replied to your voice") — a voice answered whose author never learns of it is a voice ignored.

Escalating a complaint into a corrective action

Some voices need more than a reply. The "Escalate to action" button creates a corrective action on the actions board:

  • It carries its lineage visibly on the card: "born from a worker complaint" with its subject — never the submitter's name, so escalation does not breach confidentiality.
  • A single, non-duplicating thread: pressing twice does not create two actions.
  • It takes high priority for a complaint, medium for a suggestion.
  • From that moment it enters the action lifecycle — owner, due date, verification, closure — and counts in the CAPA indicators and in the management-review inputs.

That is how a worker's voice closes its loop: said → answered → acted on → documented.

The effect on your compliance

  • ISO 45001 clause 5.4: turns green when real participation exists within twelve months — a closed survey or answered submissions. A channel that exists without answered participation stays amber.
  • The Jordanian safety guide: the worker complaints register clause reads the very same channel, and the committee-recommendation follow-up clause reads submissions left pending for more than thirty days — so a pile of ignored voices turns your clause red.
  • Management review: the "consultation and worker participation" section reads committee meetings, closed surveys, and voices received and answered automatically (see Management review).

Who sees what

Every user has "My Voice", without exception. Triage, replying and escalation belong to the organization owner, the compliance manager and the HSE officer, while authoring surveys belongs to the organization owner and the compliance manager.
